How to use this template

  1. Start by sharing the template Space with your team and brainstorm together to identify all the possible ways the project could fail.
  2. Assess each failure by evaluating its severity (impact), likelihood of occurrence, and how easily it can be detected. Then, rate each factor for every failure mode.
  3. Calculate the risk priority number (RPN) by multiplying severity, occurrence, and detection to prioritize which failures require immediate action.
  4. Develop an action plan based on the RPN, assign tasks to team members, and regularly review the template to assess effectiveness and identify new risks.
